Attending Introduction

From your attending: Internist and nephrologist with a thriving practice in IM and nephrology, both inpatient and outpatient. I am also the Medical Director for one of the two dialysis units in the area and do frequent rounds there as well.

Students can expect outpatient exposure to a mix of IM and nephrology patients. My usual patient load is 35-50 patients in the office and intermittent rounds at the hospital. We also see some of these patients virtually using telemedicine. We also go to see my dialysis patients ~ 50 of them in the dialysis centers.

Rotation Description

During your Nephrology clinical rotation with FMG Portal, you will gain valuable experience in the diagnosis and management of kidney-related conditions in an outpatient setting.

Additionally, your performance during this rotation will lead to obtaining a US LOR, which is essential for your upcoming residency program applications.

Here’s a detailed overview of what you should expect to learn. Please note that all rotations will have variations of patients and may not include the entirety of the following:

Management of Kidney Disorders

  1. Medical Management: Learning about pharmacological interventions for managing conditions like hypertension, electrolyte imbalances, and complications of kidney disease.

  2. Dietary and Lifestyle Interventions: Understanding the role of diet, fluid management, and lifestyle modifications in managing kidney disease and preventing progression.
  3. Dialysis: Introduction to various modalities of renal replacement therapy, including hemodialysis, peritoneal dialysis, and the principles of vascular access management.

Understanding Renal Physiology and Pathophysiology:

  1. Renal Function: You will learn about the normal functioning of the kidneys, including filtration, reabsorption, and secretion.

  2. Kidney Diseases: Understanding the pathophysiology of common renal disorders such as chronic kidney disease (CKD), acute kidney injury (AKI), glomerulonephritis, and nephrotic syndrome.

Acute Care Skills:

  • Admission and Discharge: Learning the process of admitting patients to the hospital, including obtaining relevant history and formulating initial management plans. Also, understanding the discharge process and planning for post-hospitalization care.

  • Monitoring: Learning to monitor and manage patients in acute settings, including interpreting vital signs, laboratory results, and imaging studies to guide treatment decisions.

  • Discussion: Engaging in discussions about differential diagnoses, treatment options, and evidence-based
    medicine with the medical team.
By the end of your Internal Medicine/Nephrology rotation, you should be well-equipped with the foundational skills needed for managing a diverse patient population in an ambulatory setting, with a strong emphasis on preventive care, chronic disease management, and patient-centered communication.

Patient Appointments:

Conducting initial and follow-up appointments, managing acute and chronic conditions, and performing routine health maintenance.
